Kkok Du is a man who lived in Korea in the past. The gods turned him into an immortal grim reaper who must come to this world every 99 years to ruthlessly punish people because of their weakness and frailty. He has come to despise people and is now cruel and inappropriate. When he came to the world around him, he "possessed" the body of a promising young doctor named Do Jin Woo.
But he finds his duty to send souls to the afterlife unsatisfying when he has to complete Do Jin Woo's medical work. This leads him to cross paths with Han Gye Jeol, a doctor who graduated from the lowest medical school in South Korea. Despite his lack of background, however, Kkok Du discovers that Han Gye Jeol has mysterious powers - and seems to have her under his spell. Did his fate interact with him in a previous life? Why is he able to do what he likes? Will he succeed in his mission - or will he be distracted by love?
